1040 Form Schedule B

Interest and ordinary dividends. This form is used for reporting interest and ordinary dividends by individual taxpayers. If an individual taxpayer has interest and / or dividend income that exceeds $400 in a tax year, then they must file a schedule b along with their Form 1040 return.
